Health and Wellness at Minneola Place
As stewards for the health, safety and welfare of our residents, we understand the seriousness of our responsibility. Minneola Place is overseen by a group of directors with strong ties to the healthcare community - physicians, nurses and therapists all advise our day-to-day management and provide education to frontline staff on how to better assist residents in managing their health. We've worked to develop close partnerships with highly-rated, high-quality healthcare providers to ensure our residents receive only the best care.
Medication Management Program
Our staff provides medication assistance and management for all our residents, including our respite care and short-stay residents. Under direction of nurse, we assist residents with maintaining compliance with a prescribed medication regimen, and we can assist with the administration of most medications.
Appointment Assistance and Coordination
We can provide assistance coordinating the health services of residents, including providing appointment reminders, helping schedule local appointments, as well as assisting with obtaining transportation to appointments.

Residents who receive medical care on-site from any home health or hospice provider of their choice will also benefit from appointment reminders, as well as help with maintaining their care records and important healthcare documents.
Primary Care with HomePoint Physicians
We partnered with HomePoint Visiting Physicians Group to provide primary care for all of our residents. HomePoint provides in-person, in-home primary care for those residents unable to see their own primary care physician. HomePoint coordinates with our staff on a daily basis to ensure the medical needs of our residents are met each day, every day - ensuring prescription continuity, regular reviews of medications, and telehealth appointments when not on-site.
HomePoint Visiting Physicians Group
Skilled Nursing and Physical Therapy with CaringPlus
We partnered with CaringPlus Homecare, LLC to provide in-home skilled nursing and therapy for our residents. When residents need nursing, physical therapy, occupational therapy, or speech therapy we coordinate with CaringPlus to meet their needs. Overseen by the primary care physician, skilled home health services are a great way to keep residents in home at the facility, but still providing high-quality nursing or therapy services.

Don't feel pressured to receive services only from our vetted healthcare partners. If you have your own healthcare providers that you would prefer us to coordinate with, let us know and we will work our hardest to ensure your health needs are still met.
